KZV-ASPTA Hot Lunch Sign-up Form

Due to popular demand, Hot Lunch will be served 3 days a week. Monday, Wednesday & Friday.

We have decided to make Monday & Wednesday Hot Lunch served to all students.

We found that almost 90% of students paid for both days so in order to prepare enough Lunch we will make both days mandatory.

Friday Hot Lunch will be an optional day. If you feel you want this day added it will be offered with a 1 time fee.

Should you decide not to participate in the Friday Hot Lunch Program; a booklet of 5 passes will be sold upon request for $25.00. You may use these passes at any time during the 2007-2008 school year.

We have decided to DECREASE the fee for Hot Lunch for Pre-school students.
